Job Title: Travel LVN / LPN – Long-Term Care (LTC)
Location: Vestal, NY 

Assignment Details

  • Duration: 16 Weeks
  • Schedule: Mid Shift | 2:30 PM – 11:45 PM
  • Hours: 5x8 | 40 hours/week
  • Weekend: Every other weekend required

Position Overview

"Navitas Healthcare, LLC" is seeking a Licensed Practical Nurse (LPN/LVN) to provide direct patient care in a long-term care setting. The role involves medication administration, monitoring resident health, and supporting patients with chronic conditions including Alzheimer’s and dementia.

Key Responsibilities

  • Administer medications safely (Med Pass)
  • Monitor patient condition and report changes
  • Provide care for dementia and Alzheimer’s patients
  • Assist with daily living activities (ADLs)
  • Maintain accurate documentation
  • Follow infection control and facility protocols
  • Collaborate with interdisciplinary care team

Required Qualifications

  • Active New York LPN/LVN License (Required)
  • Minimum 1 year of experience
  • BLS Certification
  • COVID-19 Vaccination (Required)
  • Ability to complete compliance within 1.5 weeks
  • Must commit to assigned shift

Preferred Qualifications

  • LTC experience
  • Experience with dementia/Alzheimer’s care
  • Familiarity with PointClickCare EMR
  • Background in rehab, med-surg, or hospital settings
  • First-time travelers welcome
