Summary/Position Objectives:

This position helps to develop and implement a community-based organization/social care network for the Population Health/SDoH programs at Maryland Physicians Care to improve health outcomes of MPC members. This role is responsible for fostering and retaining community partnerships for our social service network, development of strategies to address gaps specific to MPC’s population’s social needs, and analyze, track, and report data metrics for the community-based organization/social care network.

Essential Functions:

  • Develop the Community Based Organization (CBO) network strategy and implementation, specifically focused to address the MPC population/subpopulation social needs.
  • Maintain cooperative relationships with the CBO network.
  • Develop and maintain cooperative working relationships with agencies and organizations interested in addressing social needs.
  • Develop and implement referral tracking/invoicing processes and train CBO partners in processes.
  • Validate and maintain demographics information for strategic CBO partners within curated network.
  • Ensure communication activities are based on principles, methods, and best practices.
  • Use data and analyses to track and report network metrics and to identify gaps/issues within the CBO network.
  • Complete assigned contract deliverables, including required reports, within established timeframes.
  • Manage project work, as needed, including quality assurance and deliverable compliance.

Secondary Functions:

  • Work from home position. Must be available and willing to travel up to 35% of the time to meet with community-based organizations and partners, and for in-person team meetings.
  • Participate in other MPC committees as determined.
  • Observe confidentiality of health records in accordance with MPC policies and procedures.
Requirements

Knowledge and Skills:

  •  Excellent verbal and written communication skills; strong business analytical skills; strong project management skills.
  •  Experience with Microsoft Office Suite, with proven expertise in Microsoft Excel.



Education and Work Experience:

  • Bachelor’s or advanced degree in health-related field or community health or services
  • A minimum of 3 years’ recent work-related experience in public health/community-based setting or equivalent combination of education and experience.
  • Social Care Network/CBO development experience.
