Position Title: Clinic Care Coordinator – RN (Case Management)

Location: Fort Defiance, Arizona
Duration: 13 Weeks
Setting: Outpatient Surgery Clinic

Schedule

• Monday – Friday
• Shift Options:
  • 07:30 AM – 04:30 PM
  • 08:00 AM – 05:00 PM
• 8-hour shifts
• 36+ hours guaranteed per week
• No call, weekends, or holidays
• Maximum 2 call-offs during the assignment

Position Overview

"Navitas Healthcare, LLC" is an experienced Clinic Care Coordinator – Registered Nurse (RN) to support case management functions within an outpatient surgical clinic. This role focuses on care coordination, patient advocacy, and ensuring seamless transitions of care through collaboration with providers and interdisciplinary teams.

Key Responsibilities

• Coordinate and manage patient care plans in collaboration with physicians and care teams
• Perform comprehensive patient assessments and develop individualized care plans
• Facilitate care coordination across outpatient, inpatient, and specialty services
• Monitor patient progress and ensure continuity of care
• Assist with discharge planning and follow-up arrangements
• Educate patients and families on treatment plans and available resources
• Maintain accurate and timely documentation in RPMS EMR system
• Ensure compliance with facility policies and regulatory standards
• Communicate effectively with internal departments to optimize patient outcomes
• Float to other departments as needed within scope of experience

Required Qualifications

• Active Registered Nurse (RN) license
• Minimum 3 years of case management experience

Preferred Qualifications

• Experience in outpatient or clinic-based care coordination
• Familiarity with RPMS EMR system

Skills & Competencies

• Strong knowledge of care coordination and discharge planning
• Patient advocacy and care management expertise
• Excellent communication and interpersonal skills
• Strong organizational and problem-solving abilities
• Ability to manage multiple patient cases in a fast-paced environment
