POSITION OVERVIEW
• Mission-driven outpatient Internal Medicine opportunity serving a diverse adult patient population within an integrated care environment
• Practice in a coastal Northern California community surrounded by Pacific coastline and redwood forests while maintaining strong compensation and long-term financial incentives
• Collaborative, team-based outpatient model designed to support complex chronic disease management and continuity of care

• Base compensation up to $267,624 annually
• Additional 10% geographic salary differential for Crescent City location
• $12,400 annual access bonus paid biweekly for meeting patient access targets
• Additional productivity bonus potential up to $44,000 annually
• Up to $50,000 sign-on bonus plus structured retention bonuses
• NHSC and federal loan repayment eligibility ranging from $75,000 to $300,000
• Flexible schedule options including 4x10 shifts
• Strong six-month mentorship and onboarding program
• Additional relocation support available for qualifying family relocation
• Integrated support resources including behavioral health, telepsychiatry, acupuncture, chiropractic, and virtual specialty services
SCHEDULE & PRACTICE MODEL
• Flexible full-time scheduling options including 5x8 or 4x10 shifts
• Outpatient-only Internal Medicine practice
• Team-based care model with multidisciplinary support
• Dedicated support including 2 MAs and 0.7 RN support per provider
• Virtual triage and specialty collaboration resources available
COMPENSATION & BENEFITS
• Base salary: $234,635 – $267,624 annually
• 10% geographic differential for Crescent City location
• $12,400 annual access bonus paid biweekly
• Up to $44,000 annual productivity bonus potential
• Partnership-eligible track: $50,000 sign-on plus $25,000 retention bonus in years 2 and 3
• Alternative retention structure available for non-partnership track providers
• $10,000 relocation reimbursement
• Additional $10,000 relocation support for parent or in-law relocation
• 29 days paid time off including CME, holidays, and sick leave

• $5,000 annual professional expense credit for licensing, DEA, CE, and equipment

• Spanish language differential pay available
• MD or DO REQUIRED
• Board Certified or Board Eligible in Internal Medicine REQUIRED
• Active California medical license or ability to obtain REQUIRED
