Job Title: Travel Licensed Practical Nurse (LPN) – Long Term Care
Location: Falmouth
Duration: 13 Weeks
Schedule: 40–42.5 hours/week
Shift: Evening Shift (3:00 PM – 11:30 PM)
Days: 5 days/week (Every Other Weekend + Rotating Holidays)
Position Overview
"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a long-term care setting. This role involves delivering high-quality nursing care to residents, supervising support staff, and ensuring compliance with clinical and regulatory standards. The ideal candidate will demonstrate strong clinical judgment, leadership skills, and a commitment to resident-centered care.
Key Responsibilities
- Provide direct nursing care in accordance with established care plans and physician orders
- Supervise and support nursing assistants in daily resident care activities
- Administer medications and treatments safely while verifying patient identity
- Monitor resident conditions and document changes accurately in medical records
- Transcribe and implement physician orders with attention to detail
- Assist with admissions, transfers, and discharges, including required documentation
- Perform clinical procedures such as wound care, catheterization, tube feedings, and specimen collection
- Measure and record vital signs (temperature, pulse, respiration, blood pressure)
- Communicate effectively with physicians, residents, and family members regarding care plans
- Collaborate with interdisciplinary teams in care planning and delivery
- Maintain adequate supplies and report needs to the Nurse Supervisor
- Ensure adherence to infection control protocols and patient confidentiality standards
- Promote resident safety, comfort, and overall well-being
- Participate in quality improvement initiatives, surveys, and inspections
Qualifications
- Graduate of an accredited nursing program or approved LPN program
- Active, unrestricted LPN license in the state of Massachusetts
- Current BLS / CPR / AED certification required
- Prior experience in long-term care preferred
- Strong knowledge of nursing practices, procedures, and regulatory standards
- Ability to supervise staff and make sound clinical decisions independently
- Excellent communication and interpersonal skills
- Ability to work collaboratively with residents, families, and healthcare teams
- Must be able to read, write, and communicate effectively in English
- Physically capable of lifting up to 50 lbs and meeting job demands
