Job Type: Contract (Local or Travel)
Duration: 16 Weeks
Shift: Evening Shift | 2:30 PM – 11:45 PM

Position Overview

"Navitas Healthcare, LLC" is seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) for a 16-week contract assignment in a Long-Term Care / Skilled Nursing Facility setting. The LPN will provide direct patient care under the supervision of a Registered Nurse (RN) or physician, ensuring high-quality care and patient safety in a fast-paced environment.

Key Responsibilities

  • Perform focused patient assessments and report any changes in condition to the RN or healthcare provider
  • Administer medications, treatments, wound care, and IV therapies in accordance with scope of practice and facility guidelines
  • Assist in the development and implementation of individualized patient care plans
  • Collaborate with RNs, physicians, and interdisciplinary teams to support patient care and discharge planning
  • Accurately document patient care activities, observations, and responses in the EMR system
  • Provide basic patient and family education as directed by the RN
  • Respond promptly to patient needs and communicate effectively with the healthcare team
  • Maintain compliance with infection control protocols, medication administration standards, and safety regulations
  • Assist with patient admissions, transfers, and discharges
  • Support unit operations and contribute to a positive team environment

Qualifications

  • Active Licensed Practical Nurse (LPN) license in the state of New York
  • Basic Life Support (BLS) certification required (AHA preferred)
  • Minimum 1 year of recent LPN experience in a hospital or long-term care setting preferred
  • Strong knowledge of basic nursing practices and clinical procedures
  • Ability to work effectively under supervision in a team-based environment
  • Proficiency in EMR systems and standard documentation practices
  • Excellent communication, time-management, and organizational skills
