Job Summary:
"Navitas Healthcare, LLC" is seeking a dedicated Mental Health Coordinator to provide clinical support and coordination of mental health services. The ideal candidate will have strong clinical expertise and experience in delivering patient-centered care while ensuring compliance with professional standards and regulations.
Key Responsibilities:
- Conduct mental health assessments, counseling, and therapeutic interventions
- Develop and implement individualized treatment plans
- Coordinate care with multidisciplinary teams to ensure continuity of services
- Monitor patient progress and adjust treatment plans as needed
- Maintain accurate and timely clinical documentation
- Ensure compliance with ethical standards, policies, and regulatory requirements
- Provide crisis intervention and support as necessary
- Educate patients and families on mental health conditions and treatment options
Required Qualifications:
- Master’s degree in Social Work, Psychology, Mental Health Counseling, or related clinical field
- Active and valid LPC, LCSW, or LMFT license in Colorado
- Must be at least 18 years of age
